You by now have heard the story of the irresponsible individual with terrible resistant tb who traveled around the globe by jet airplane perhaps infecting others. Just what is this resistant tb disease and why is it so dangerous.

First of all this problem of spread of Tuberculosis by people who are infectious with tb is not a new phenomenon. It has happened before. Only now it is becoming more frequent and common with both the rise and spread of new hard to treat resistant forms of tb and the reemergence of tb which is associated with aids.

TB is a deadly disease caused by a bacterium that the body just cannot seem to get rid of. The body tries and tries to get rid of the Mycobacterium tuberculosis. Bacteria by the immune system again and again more and more but it cannot. In the process the potent chemicals of the immune system destroy parts of the body. It as if strong drain cleaner is being poured on your floor again and again. Only in this case it is parts of your body – usually your lungs. Any part of the body can be affected as well as the lungs – bones, joints, circulatory system evens the skin.

As with most diseases Tuberculosis (TB) originated with man’s involvement with the domestication of animals. Influenza came from our involvement with fowl such as ducks and chickens; TB comes from man’s involvement with cattle.

Remarkably one third (1/3) of the world’s population has tuberculosis. Most of these people are no threat to anyone at all. TB has become much more common. TB had been said not long ago to be “cured” and there was even great effort to close major tb major tb centers as the National Jewish Hospital in Denver (where the world TB traveler was eventually relegated and treated). TB was rare and just about unknown. Then came the aids epidemic. Aids and tb seem to go hand in hand.

With the aids epidemic came multi drug resistant tb. What that means is that the drugs that used to treat and eradicate the tb became less and less effective and in many cases cannot even touch the tb. Whether this is because of the bug changing, the fact that many of the aids patients in the us were intravenous drug users or whatever no one knows.

The basic problem is that although 90 % of Tuberculosis patients don’t spread the disease and are no problem is not the issue. It is the remaining 10 % who are active carriers of the disease are. One tenth of those infected by TB will die if left untreated. And now a greater and greater percentage of Tuberculosis can not be cured by the old standby drugs. Tuberculosis was a difficult disease for both the doctor and patient to treat – lots of different pills that not only cured the Tuberculosis but also had lot of side effects. As well it took a long time period of the multi antibiotic therapy. It was not just a simple 10 day treatment and all was over.

That person who is actively spreading tb can infect 10 to 15 other people a year. How can you prevent the spread of TB to yourself? First of all avoidance of the person who has active tb. TB is generally spread by the aerosol droplets sneezed or spit by the carrier person. Secondly make sure any utensils – plates, knives forks etc that you use are carefully sterilized. A hot dishwasher will do that.

I Have Rheumatoid Arthritis And Have Heard About Cimzia… What Is It?

Biologic therapy has revolutionized our approach to the treatment of rheumatoid arthritis (RA). Less than 10 years ago, the best we could hope for was to “modify disease” or slow it down and also help with symptoms. Now the goal is to not only control symptoms, it is to get RA into complete remission. Biologics are protein-based medicines that are synthesized in a laboratory. They act like laser beams to target the immune abnormalities that are felt to cause RA.

First generation biologics such as etanercept (Enbrel), infliximab (Remicade), and adalimumab (Humira) are known as TNF-inhibitors and have done wonders for many patients. Second generation biologic such as rituximab (Rituxan) which acts against B cells and abatacept (Orencia) which works on T cells are both welcome additions to the arsenal of weapons available to combat RA.

Cimzia (certolizumab) is an investigational TNF-inhibitor. It differs from the current crop of TNF-inhibitors since it is “pegylated.” This means that a substance called polyethylene glycol has been attached to the molecule. This pegylation lengthens the half-life of the drug- meaning the drug stays in the system longer. Cimzia also has had a piece of protein removed from the molecule. The piece of protein that has been removed contained a small amount of mouse protein (yes… these drugs are often created using mouse proteins). By removing the piece of mouse protein, it is hoped that Cimzia will cause fewer adverse reactions.

A recent presentation on June 14, 2007 at the annual meeting of the European Congress of Rheumatology (EULAR) described data on Cimzia.

The team studied 2 dose regimens, which patients received subcutaneously as add-on therapy to methotrexate.

In a phase 3, multicenter, double-blind, placebo-controlled, parallel-group study, the investigators recruited 992 patients with RA who received either the study drug or placebo. The investigators wanted to know the rate at which people had a 20% improvement, as defined by the American College of Rheumatology criteria (ACR 20).

The patients, who had previously been treated for at least 6 months with methotrexate, were randomized to treatment with either pegylated certolizumab or placebo. Those on treatment received 3 400-mg doses every 2 weeks, followed by 200 or 400 mg doses of certolizumab pegol every 2 weeks. The patients continued methotrexate as usual. The investigators assessed the efficacy and safety parameters at 2-week intervals.

In an early analysis at 24 weeks, the investigators found that 581 patients completed the study: 259 of the 397 on 200 mg of the drug, 278 of the 394 on 400 mg, and 44 of the 201 on placebo. The ACR20 response rate was 59.2% in the 200 mg group, 61.2% in the 400 mg group, and 13.5% of those who received methotrexate and placebo. The proportion of patients who experienced a significant side effect was 74.0% in the 200 mg group, 76.1% in the 400 mg group, and 57.7% in the placebo group. The majority of adverse events were mild to moderate.

“Pegylated certolizumab adds significant benefit in reducing the signs and symptoms of RA in combination with methotrexate, compared to using methotrexate alone,” said lead investigator Edward C. Keystone, MD, professor of medicine at the University in Toronto, and director of the Rebecca MacDonald Centre for Arthritis and Autoimmune Disease, in Ontario.

Bottom line: Pegylated certolizumab works like a TNF blocker and appears to treat the signs and symptoms of RA. Whether it’s better than the TNF blockers that are currently available is still no known. It is better than placebo.

The Inside Story Of Acne Cleansers – 1

Acne cleanser is used to prepare the skin before application of topical acne treatment- a cream, gel or a lotion. Ideally any acne cleanser should remove all the surface oils, dirt and any other impurity from the skin. Once the skin is clean the medication becomes more effective because there is no barrier between the medicine and the skin cells. Let us look at the common acne cleansers available and find out what you should be looking for.

Look for a gentle cleanser. It should be non-irritating and acid balanced. The cleanser should be non comedogenic and hypoallergenic. Let me explain these terms in detail.

Hypoallergenic-

Any product that does not cause allergy by use is called a hypoallergenic product. By reading the word hypoallergenic on any product, most people believe that it will not cause any allergy in them. But this may not be true. Manufacturers are not required to substantiate their claim that their product is hypoallergenic. The use of this term by a reputed company may mean that a product labeled as hypoallergenic has less chances of causing allergy in most of the consumers. You have to use a product and find out if it causes any allergy. Use products marked as hypoallergenic, etc., with caution because the results may be interpreted by a company in its favor to sell more products by luring consumers. Still a product marked hypoallergenic is better than a product that is not marked so.

Non comedogenic-

You might have observed that when you begin applying a particular product for some time, you begin getting comedones- whiteheads and blackheads. Any ingredient that can cause comedones is called comedogenic. Any product that does not cause comedones is called non-comedogenic.

To find out if any material is comedogenic, it is usually tested on the insides of a rabbit’s ear. After three weeks, the area of application is observed and a small biopsy is done. The numbers of comedones are counted and the material tested is labeled as comedogenic on a scale of 0 to 5. The material that causes no comedogenicity is labeled 0 and the highest comedogenic product is labeled as 5.

This test is carried out on individual materials, for example – mineral oil, or calendula or cetyl alcohol, etc. Every product that is to be used on the body can be tested in this manner and labeled. When you use a product for application on your body, you will find that most of the formulated products contain more than one ingredient. Such products are labeled as comedogenic, if any comedogenic ingredient is used in it. If a product is labeled as non-comedogenic, it is not supposed to contain any comedogenic material in it.

All of us are different and our body reacts differently to every material. Some of us may not have any comedones, even if we use a comedogenic product. Similarly, some of us may get comedones by using the safest of products. It is like asking – why I don’t get any acne, whereas my friend of same age cannot get rid of it? While using any product, read and note if it is labeled as non-comedogenic.

In the first part of this article, we found out about the qualities of an acne cleanser and the meaning of hypoallergenic and non-comedogenic products. In this part we will find out about the active ingredients that are used in acne cleansers.

First of all, let us find out why acne cleansers are called so? How are they different to other cleansers? There is not much difference between an acne cleanser and an ordinary cleanser. Both clean the skin. It may be called a marketing strategy to call them acne cleansers. Many companies have begun adding active ingredients that fight acne to the acne cleanser. The common products that are added are- Salicylic acid, Glycolic acid and Benzoyl Peroxide.

It is important to know that either these added ingredients will get washed away with the cleanser or they will remain on the skin. The effectiveness of these added ingredients to acne cleansers is debatable. Let us learn more about the ingredients that are commonly added.

Salicylic acid-

Salicylic acid is a Beta Hydroxy acid. Salicylic acid softens the keratin, which is the primary protein that forms skin, hair and nails. By breaking down keratin, salicylic acid makes it easy to remove unwanted skin cells. It is used in very small quantities to acne cleansers and may cause irritation to some people.

Glycolic Acid-

Glycolic Acid is one of the most commonly used Alpha Hydroxy Acids in skin care. It is derived naturally from sugarcane juice. Glycolic acid has a very low molecular size. The low molecular size allows glycolic acid to penetrate skin faster. The dead skin cells that bind with the skin are removed by glycolic acid by loosening the glue that sticks the dead cells with the skin. As the glue loosens, the dead cells exfoliate giving a cleaner look to the skin. Shedding dead cells faster also allows growth of new cells. This acid also helps in treatment of blackheads and dries out the excess surface oil on the skin. It is thought that glycolic acids stimulate collagen production to some extent. This acid also keeps the skin moisturized by keeping water bound to the skin and prevents drying of skin. It can also bleach some spots on the skin. One added advantage of glycolic acid is that it helps other skin care ingredients reach deeper in the skin and makes them more effective.

Benzoyl Peroxide

Benzoyl Peroxide is commonly used for treatment of acne. Benzoyl Peroxide removes excess oil, unclogs the closed pores, and kills the bacteria. Benzoyl Peroxide may irritate the skin.

Before you decide to use any acne cleanser, which is added with any of these ingredients, you should note that you would be applying your acne medication after cleansing the skin. Your acne treatment may contain one of these ingredients. You should also note that most of the active ingredient would be in contact with your skin till the acne cleanser is washed away.

Isotretinoin – It Treats The Most Severe Acne

1. Isotretinoin is considered to be most effective medicine for severe acne. Isotretinoin is a drug of choice for acne patients who may have lost hope about cure of their acne. It is available in form of capsules to be taken orally and the trade name of Isotretinoin in the USA is Accutane.

2. Isotretinoin is a retinoid. Retinoids are derived from Vitamin A. Isotretinoin is chemically known as 13-cis retinoic acid.

7. In the beginning some people see deterioration in their acne but that slowly disappears. Isotretinoin is commonly prescribed for 16 to 20 weeks and if needed it is repeated after a gap of eight weeks.

8. Nodular and cystic acne that does not get treated with other medications is treated with Isotretinoin. Any other acne that does not respond to medication is also treated with Isotretinoin.

9. As Isotretinoin needs fat to help its absorption in body, it is taken after food. Please follow your doctors advise about these.

10. Please note that you may not be able to wear your contact lenses for some time in the beginning of treatment with Isotretinoin. You are also not supposed to donate any blood while you are taking Isotretinoin and for one month after you stop taking it. Please talk about dos and do not with your doctor.
